Use the request from your insurer or adviser to identify the records you need translated. A claim packet may contain an accident report, receipts, repair records or documents concerning property or travel. Send the selected files and explain which recipient has requested them.

Keep the claim references together
Label the files with the claim reference you already use and identify the person or event each record concerns. Keep attachments with their source document. Dates, receipt numbers and related references can be important when several records describe different parts of the same incident.
Include complete pages with headings, stamps and handwritten notes visible. If a scan is unclear or a page is missing, mention that in your request. Point out discrepancies in the originals without editing them to match; the translation should reflect the supplied records rather than investigate or settle the facts of the claim.
Medical evidence within a larger packet
A wider insurance packet may include medical records as well as financial or incident documents. Identify those records separately and include the instructions for the organization receiving them. Keep clinical questions with the treating professional and coverage questions with the insurer. Explain any required language or certification details as part of the document request.
