Documents for your receiving office

Identify the clinic, legal adviser, court or other office receiving your translated records. Send the documents that recipient has selected, which may include supplied parentage, birth or court records. Include the relevant written instructions so the quote can address the actual packet and language requested.

Medical records within the packet

If your packet includes medical records, label them separately from legal or civil documents. Include complete pages, headings and any attached notes that are part of the source. Describe the recipient and requested language without assuming that every file has the same purpose.

Names and instructions for each record

Supply the name spellings already used by the receiving office and identify which person each record concerns. Where names appear in another script, reference spellings can help explain how they are used in the rest of your packet. Keep dates and document references visible in the source copies.

Flag differences between originals without altering the files to make them agree. Translation should reflect the supplied information, not establish parentage or authenticate the source record. If an adviser has given instructions for labels or attachments, include those separately and make clear which documents they concern.
