Polish medical records translation for a new doctor in the US, a disability or injury claim, an insurer or ECFMG. We translate the karta informacyjna leczenia szpitalnego, the epikryza, each wynik badania and the full dokumentacja medyczna from Polish hospitals and przychodnie.
A Polish hospital gives the patient a karta informacyjna on discharge, and everything else sits in the history kept by the ward or the clinic. Each piece has its own vocabulary.
Karta informacyjna leczenia szpitalnego
This discharge card is the document most US doctors need. It opens with the rozpoznanie and its ICD-10 code, which carries over unchanged while the Polish wording is translated, then the epikryza, a narrative of the stay, the wyniki badań, the zastosowane leczenie and the zalecenia with dosing for home. We keep the headings in that order. The card is signed with the doctor's pieczątka, which gives the name, the specialty and the seven-digit numer prawa wykonywania zawodu, and the patient's PESEL is printed on every page.
Shorthand a US reader cannot decode
Polish doctors write w wywiadzie for a history of, bz for bez zmian, no change, st. po for stan po, status post, p.o. for by mouth, tabl. for a tablet, amp. for an ampoule, x dz. for times a day and r.ż. for rok życia, the patient's age. Each one is written out in English. Lab values use a decimal comma and dates run 03.07.2024; some Polish labs report glucose and cholesterol in mmol/l and others in mg/dl, and the translation keeps each value and unit exactly as printed.
Sick leave, disability and clinic papers
Beyond the hospital there is the e-ZLA, the electronic sick note filed with ZUS, the orzeczenie o niepełnosprawności from a powiatowy zespół with its stopień of lekki, umiarkowany or znaczny, the orzeczenie of a lekarz orzecznik ZUS on niezdolność do pracy, and the skierowanie that got the patient to a specialist. Records downloaded from Internetowe Konto Pacjenta print as PDFs with the issuing clinic in the header, and we translate those as readily as a stamped paper.
Who is asking for your Polish medical records translation?
The translation is the same. What changes is whether the recipient wants anything added to it.
Doctors and hospitals
A new physician needs your diagnoses, medications and reports in English before the first visit. Each report is translated in full and certified, so it can go into your chart as it is.
Health, travel and disability insurers ask for the medical report and the itemized bill in English, translated completely and certified, before they pay a claim for care received abroad.
ECFMG accepts a word-for-word translation on a translation service's letterhead, signed by the translator with a certification statement. Summaries and extracts are refused. Our certified translations follow that format.
Three things that save a day of back and forth on a Polish medical records.
01
Send the pages a doctor would read
The discharge letter, the current medication list and the reports it relies on cover most visits. A full hospital file can run to hundreds of pages, so start with those and add the rest if the recipient asks.
02
Letterhead, stamps and signatures count
The hospital header, the doctor's stamp and the registration number on a prescription identify who wrote the record. Scan each page whole, including forms and the back of any page with a stamp.
03
Tell us who will read it
A physician needs the clinical pages, an insurer the diagnosis and the itemized bill, ECFMG a word-for-word translation of the entire document. Say which in the order notes, with the patient's name as the passport spells it.
Details and pricing
What is included in a Polish medical records translation
One price per page covers the translation and its certification. A page is up to 250 words, and the quote tells you the page count before you pay.
Everything on the medical records is translated, including stamps, seals and handwritten notes. Tell us the English spelling you use for each name when you order.
Which pages should I translate for a doctor in the US?
Start with the most recent karta informacyjna, your current list of leki, and the results the card relies on: histopatologia, a TK or MR report, the latest lab panel. A full history from a przychodnia can run to dozens of visits, and most US doctors want the summary before the detail. Add the records from an oddział ginekologiczny or neonatologiczny only when the visit is about that.
Can you translate Polish records for an injury or insurance claim?
Yes. A protokół powypadkowy records a work accident, the e-ZLA proves the sick leave, an opinia biegłego lekarza gives the expert view a court case turns on, and a karta wypadku comes from ZUS. For a travel insurance claim we also translate the faktura from a private clinic, which lists each service by name; the amount in złoty stays and the description is translated.
Do you translate US medical records into Polish?
Yes. People send US hospital records to ZUS for a renta or a disability decision, to NFZ when claiming for treatment received abroad, to PZU or another Polish insurer, or to their lekarz rodzinny at home. Tell us who will read the file, because a lekarz orzecznik ZUS and a family doctor expect different terms for the same finding, and we pick the translator accordingly.
How much does a certified translation cost?
Certified translation is $25 per page, with a page counted as up to 250 words. Notarization, a mailed physical copy and an apostille are optional and priced separately.
How long does a certified translation take?
Standard turnaround starts at 24 hours and depends on the length and language of your documents. If you have a deadline, include it in your quote request.
