What a Russian hospital hands you at discharge

A Russian patient leaves the hospital with a выписной эпикриз and whatever the doctor stapled to it, and most of what a US physician needs is in that one document.

Выписной эпикриз and форма 027/у

The выписной эпикриз is the discharge summary, written for the поликлиника doctor who takes over. It opens with the клинический диагноз split into основной, сопутствующий and осложнения, gives the МКБ-10 code, then жалобы, анамнез, the course of treatment and рекомендации with the drugs to continue. The shorter выписка из медицинской карты on форма 027/у covers a clinic visit or a run of them. We keep the headings in the same order, so the US doctor reads основной as principal diagnosis and сопутствующий as comorbidity.

Abbreviations and diagnoses with no US name

Russian doctors write ОАК for a complete blood count, ОАМ for urinalysis, б/х for the chemistry panel, АД 130/80 for blood pressure, ЧСС for heart rate, ИБС for ischaemic heart disease, ГБ for hypertension, ОРВИ for a viral respiratory infection. Each is written out in English. Some diagnoses are common in Russia and absent from US practice: ВСД, вегетососудистая дистония, and остеохондроз used for ordinary back pain. The translation gives the literal term with a short bracketed note rather than substituting a US diagnosis the doctor never made.

Stamps, the МСЭ certificate and sick leave

A Russian medical paper carries a round печать of the hospital, a triangular stamp reading для справок и больничных листов, and the doctor's личная печать with name and specialty, and each one is translated with the office it names. A справка МСЭ from the Бюро медико-социальной экспертизы states a disability group, I, II or III, and the translation keeps the group number with a note that I is the most severe, the reverse of what a US reader may assume. Sick leave is a листок нетрудоспособности, electronic since 2022.

Who is asking for your Russian medical records translation?

The translation is the same. What changes is whether the recipient wants anything added to it.

ECFMG translation requirements

Before you upload

Three things that save a day of back and forth on a Russian medical records.

01

Send the pages a doctor would read

The discharge letter, the current medication list and the reports it relies on cover most visits. A full hospital file can run to hundreds of pages, so start with those and add the rest if the recipient asks.

02

Letterhead, stamps and signatures count

The hospital header, the doctor's stamp and the registration number on a prescription identify who wrote the record. Scan each page whole, including forms and the back of any page with a stamp.

03

Tell us who will read it

A physician needs the clinical pages, an insurer the diagnosis and the itemized bill, ECFMG a word-for-word translation of the entire document. Say which in the order notes, with the patient's name as the passport spells it.

Most questions about pricing and turnaround can be answered faster by requesting a quote.

Request a Quote

Which pages should I send for a new doctor in the US?
The latest выписной эпикриз, the results it relies on, УЗИ, КТ or МРТ reports and the most recent blood panel, and your current prescriptions. A full амбулаторная карта from a поликлиника is handwritten across years of visits and can run to a hundred pages, each page of up to 250 words at $25, so start with what the doctor will read. Lab values stay in the units printed, usually mmol/l and g/l, and dates are written out so 03.07.2024 cannot be read as March.
My US doctor wants diagnosis codes. Can you add them?
We keep any МКБ-10 code the Russian doctor wrote, since it is the WHO ICD-10 code and a US coder can map it to ICD-10-CM. We do not add codes the doctor never wrote, because choosing a code is a clinical decision and a certified translation cannot make it. If the эпикриз gives only words, the translation gives the diagnosis in English words.
Do you translate US medical records into Russian?
Yes. People send US records home when treatment continues in Russia or Kazakhstan, when a Russian insurer or employer asks for proof of treatment, or when the Бюро МСЭ reviews a disability that began abroad. A translation into Russian gives metric figures beside imperial ones, so weight in pounds carries kilograms next to it, and US brand names of drugs carry the active ingredient a Russian pharmacist will look for.
How much does a certified translation cost?
Certified translation is $25 per page, with a page counted as up to 250 words. Notarization, a mailed physical copy and an apostille are optional and priced separately.
How long does a certified translation take?
Standard turnaround starts at 24 hours and depends on the length and language of your documents. If you have a deadline, include it in your quote request.
