Korean medical records translation for a US doctor, an insurer, an injury claim or a visa medical: the 진단서, 소견서, 진료기록부, 수술기록지 and 처방전 from Korean hospitals and clinics, translated into English and certified. We also translate US records into Korean for a hospital or an insurer in Korea.
A Korean hospital does not hand over one file. It issues separate certificates for separate purposes, and the US reader needs to know which one is in front of them.
진단서: a certificate on a statutory form
The 진단서 is a medical certificate on a form fixed by the 의료법 시행규칙: the patient's name and 주민등록번호, the diagnosis with its 질병분류기호 under the Korean Standard Classification of Diseases, which is built on ICD-10, the date of onset, the date of diagnosis, the treatment given and the opinion on further treatment, the doctor's 면허번호 and the hospital seal. The 상해진단서 for an injury adds how it happened and a line such as 전치 4주, about four weeks of treatment expected, which an insurer or a court reads as the severity. We translate the KCD code with its description so a US clinician can match it to ICD-10.
소견서, 진료확인서 and 입퇴원확인서
A 소견서 is a doctor's opinion letter, less formal than a 진단서, often written to refer a patient to another hospital or to explain a condition to an employer. A 진료확인서 only confirms that you were seen on a date. An 입퇴원확인서 gives the admission and discharge dates. US recipients treat these differently, so the translation names each document for what it is instead of calling all of them a medical certificate.
Chart notes, 처방전 and the itemized bill
The 진료기록부 and 수술기록지 from a large hospital such as 서울대학교병원, 삼성서울병원 or 서울아산병원 mix Korean with English medical shorthand and drug names, and a released copy is stamped 원본대조필, compared with the original. A 처방전 lists each drug with 1일 3회 for three times a day, 식후 30분 for 30 minutes after meals, 정 for tablet and the number of days. Dates print as 2024. 3. 15. The translation writes the dosing out in full, keeps the generic drug name and spells the month, and the 진료비 세부내역서, the itemized bill, is translated line by line when an insurer is paying.
Who is asking for your Korean medical records translation?
The translation is the same. What changes is whether the recipient wants anything added to it.
Doctors and hospitals
A new physician needs your diagnoses, medications and reports in English before the first visit. Each report is translated in full and certified, so it can go into your chart as it is.
Health, travel and disability insurers ask for the medical report and the itemized bill in English, translated completely and certified, before they pay a claim for care received abroad.
ECFMG accepts a word-for-word translation on a translation service's letterhead, signed by the translator with a certification statement. Summaries and extracts are refused. Our certified translations follow that format.
Three things that save a day of back and forth on a Korean medical records.
01
Send the pages a doctor would read
The discharge letter, the current medication list and the reports it relies on cover most visits. A full hospital file can run to hundreds of pages, so start with those and add the rest if the recipient asks.
02
Letterhead, stamps and signatures count
The hospital header, the doctor's stamp and the registration number on a prescription identify who wrote the record. Scan each page whole, including forms and the back of any page with a stamp.
03
Tell us who will read it
A physician needs the clinical pages, an insurer the diagnosis and the itemized bill, ECFMG a word-for-word translation of the entire document. Say which in the order notes, with the patient's name as the passport spells it.
Details and pricing
What is included in a Korean medical records translation
One price per page covers the translation and its certification. A page is up to 250 words, and the quote tells you the page count before you pay.
Everything on the medical records is translated, including stamps, seals and handwritten notes. Tell us the English spelling you use for each name when you order.
Korea counted a newborn as one year old and added a year every New Year until June 2023, when the 만 나이 international age became the legal standard. Older records, and some forms still, write 만 before the age to show the international count. We keep the figure and add a translator's note when a record uses the older count, so the age on the translation agrees with the date of birth.
What should I send for a US insurance or injury claim?
The 진단서 or 상해진단서, the 입퇴원확인서, the 진료비 세부내역서 and the 영수증 receipts. If the adjuster wants the treatment history, add the 진료기록부 pages for those dates. Each page of up to 250 words is $25, and a claim usually turns on the certificate, the dates and the bills rather than the full chart.
I was treated in the US. Will my Korean 실손보험 insurer want the records in Korean?
Often. Patients continuing treatment at a Korean hospital, or claiming overseas treatment on a Korean 실손의료보험 policy, send US discharge summaries, operative reports and bills. Korean doctors read English records, but 보험사 claims departments and 국민건강보험공단 often want Korean. We translate into Korean and can notarize the translation.
How much does a certified translation cost?
Certified translation is $25 per page, with a page counted as up to 250 words. Notarization, a mailed physical copy and an apostille are optional and priced separately.
How long does a certified translation take?
Standard turnaround starts at 24 hours and depends on the length and language of your documents. If you have a deadline, include it in your quote request.
