Ukrainian name change translation of the свідоцтво про зміну імені a ДРАЦС office issues, for a USCIS file, a Social Security record, a DMV or a university that holds documents in two names, and of a US court order into Ukrainian for the consulate that must reissue the passport. In Ukraine a name is changed at the registry, not in court, and the paper trail is short.
How a name changes in Ukraine, and the documents that prove it
The registry office changes names, divorce changes a surname on its own record, and some apparent name changes are only a change of language.
The свідоцтво про зміну імені
A person of 16 applies at the ДРАЦС office, and at 14 with the parents' consent. The office checks with the police, then makes an актовий запис про зміну імені and issues a свідоцтво про зміну імені showing the old and new surname, given name and patronymic and the date, after which the passport has to be exchanged within a month. The patronymic is not chosen freely; it follows the father's given name and changes only when his name changes or through adoption. A Soviet-era change came as a свидетельство о перемене фамилии, имени, отчества in Russian, and we translate those too.
A surname taken back at divorce
A spouse who wants the earlier surname back says so when the divorce is registered, and the свідоцтво про розірвання шлюбу records the surname each former spouse carries after the divorce. No separate name change certificate exists for that, nor for a surname taken at marriage, and a US office asking for one is asking for something Ukraine does not issue. We translate the certificate that carries the change and the translator's note points to the line, so the SSA clerk can see where the new surname comes from.
Mykola and Nikolay are one name, not a name change
A Soviet certificate in Russian gives Николай, a Ukrainian passport gives Микола, and a diploma from 2005 romanizes it one way while a passport from 2019 does it another. None of that is a name change, and no Ukrainian office will issue a certificate for it. What exists is the translator's footnote tying the forms together, and for a stubborn office a notarized statement that the two names belong to one person, which we translate into English or Ukrainian as needed. The transliteration table changed in 2010, so an older passport may spell Ганна as Ganna and Юлія as Iuliia where a newer one has Hanna and Yuliia.
Who is asking for your Ukrainian change of name translation?
The translation is the same. What changes is whether the recipient wants anything added to it.
USCIS
USCIS asks for a complete English translation and a signed certification from the translator. No notarization is needed.
Offices updating your record to the new name accept a certified translation of the order, read next to the original. Some DMVs want it notarized as well, so check with your office first.
Three things that save a day of back and forth on a Ukrainian change of name.
01
Send the court order or the registry act
A name change is proved by the judge's order, the deed poll or the registry entry that recorded it. Upload the document that made the change, not only an ID in the new name.
02
Show both names in the scan
The page that lists the old name and the new one is the page every recipient wants. Include the seal, the registration number and the back of the page.
03
Give us the English spelling of both names
Tell us how each name appears on your passport or other documents. The translation keeps the names as the order writes them and notes the spelling you use.
Details and pricing
What is included in a Ukrainian change of name translation
One price per page covers the translation and its certification. A page is up to 250 words, and the quote tells you the page count before you pay.
Everything on the change of name is translated, including stamps, seals and handwritten notes. Tell us the English spelling you use for each name when you order.
I changed my name in a US court. How do I get a Ukrainian passport in the new name?
Through the consulate, which performs the registry function for Ukrainians abroad. It registers the change on the strength of the US court order, apostilled by the state and translated into Ukrainian, and then takes the application for a new passport. The order has no patronymic, and the consulate keeps yours as it stands unless you ask about it. Check with the consulate whether it wants the translation certified by its own officer rather than by us.
My child's surname was changed when I remarried. What document shows it?
Not a name change certificate. Under the Family Code a child's surname is changed at ДРАЦС on both parents' application, or on one parent's with a conclusion from the орган опіки та піклування, and the registry amends the birth record and issues a new свідоцтво про народження in the new surname. For a US file we translate the new certificate and a витяг from the register that shows the amendment, which is the document that explains why the surname differs from the earlier one.
I lost the name change certificate. What can I translate instead?
A витяг з Державного реєстру актів цивільного стану громадян about the name change record, which any ДРАЦС office issues and which a relative in Ukraine can request for you, or a повторне свідоцтво issued as a duplicate. Both carry the old and the new name and the date of the original record. We translate either in full, duplicate marking included, so the dates read correctly against your passport and your older documents.
How much does a certified translation cost?
Certified translation is $25 per page, with a page counted as up to 250 words. Notarization, a mailed physical copy and an apostille are optional and priced separately.
How long does a certified translation take?
Standard turnaround starts at 24 hours and depends on the length and language of your documents. If you have a deadline, include it in your quote request.
