Polish vaccination record translation for a school nurse, a civil surgeon completing Form I-693, a university health office or an employer. We translate the szczepienia pages of the książeczka zdrowia dziecka, the karta uodpornienia from your przychodnia and the vaccination printout from Internetowe Konto Pacjenta, dose by dose.
Poland keeps two records of the same shots: the parent's booklet and the clinic's card. Which one you hold decides what we translate and how legible it is.
Książeczka zdrowia dziecka and karta uodpornienia
The booklet is handed to parents at the maternity ward, and the szczepienia pages are filled in by hand by the clinic nurse: date, vaccine name, seria (the lot number) and the doctor's pieczątka. The karta uodpornienia is the clinic's own card, and if the booklet is lost the przychodnia will issue a zaświadczenie o szczepieniach copied from it. Doses given in recent years also appear on Internetowe Konto Pacjenta, and that printout is the cleanest record of the three.
Polish vaccine names against the CDC schedule
The Program Szczepień Ochronnych, published each year by the Główny Inspektorat Sanitarny, uses Polish names. WZW B is hepatitis B. DTP is błonica, tężec, krztusiec. Odra, świnka, różyczka is MMR, given at 13 to 15 months and again at 6 years. Polio appears as IPV, or OPV in records from before 2016. Pneumokoki (PCV) became mandatory in 2017 and rotawirus in 2021. The translation gives the US name with the Polish abbreviation in brackets so a nurse can match each line to the CDC table.
BCG, and the shots Poland does not give
Every Polish child gets BCG at birth. It is not on the US schedule, it explains a positive TB skin test, and a civil surgeon who sees it on the translated record can order a blood test instead. The reverse gap matters too: ospa wietrzna (varicella) and WZW A (hepatitis A) are szczepienia zalecane, paid for by the parents, so most Polish records lack them and a US school will ask for the shot or a titer. We mark each dose exactly as written and never infer a missing one.
Who is asking for your Polish vaccination record translation?
The translation is the same. What changes is whether the recipient wants anything added to it.
USCIS
USCIS asks for a complete English translation and a signed certification from the translator. No notarization is needed.
Most ask for a certified translation they can read next to the original. Digital delivery as a PDF is usually enough; ask for a hard copy if they want one.
Three things that save a day of back and forth on a Polish vaccination record.
01
Photograph each page flat and in daylight
Vaccination cards are small and often handwritten. A flat, well-lit photo of every page, including the cover, lets the translator read each date and lot number.
02
Tell us whose record it is
Children's cards often carry a nickname or no surname. Give us the full name as it appears on the passport so the translation matches the rest of the file.
03
Leave vaccine names and lot numbers to us
Trade names, lot numbers and stamps are copied exactly as written, and the headings around them are translated, so a clinic or school can read the schedule without guessing.
Details and pricing
What is included in a Polish vaccination record translation
One price per page covers the translation and its certification. A page is up to 250 words, and the quote tells you the page count before you pay.
Everything on the vaccination record is translated, including stamps, seals and handwritten notes. Tell us the English spelling you use for each name when you order.
The entries are handwritten and the dates look wrong. How are they translated?
Polish records write the day first, so 05.03.2016 is 5 March. The translation writes the month in words and keeps the figures beside it. A seria is a lot number and stays as printed. Photograph each page flat in daylight with the whole page in frame; our translators read the nurse's hand, and anything that truly cannot be read is marked illegible rather than guessed.
Do I need my COVID vaccination translated?
Usually not. The Unijny Certyfikat COVID that Poland issued through IKP and the mObywatel app is printed in Polish and English, so most US recipients read it as is. If all you have is the paper card from the punkt szczepień, or the dose is only in the booklet, we translate that page like any other, with the vaccine name and the seria carried over.
Can you translate a US immunization record into Polish for a Polish school or clinic?
Yes. A family moving to Poland brings the US record to the przychodnia, which copies the doses into a new karta uodpornienia and schedules whatever the Program Szczepień Ochronnych still calls for. Some gminy ask for proof of the mandatory vaccinations when a child enrols in a public żłobek or przedszkole. We translate the US record into Polish with the CDC names kept in brackets.
How much does a certified translation cost?
Certified translation is $25 per page, with a page counted as up to 250 words. Notarization, a mailed physical copy and an apostille are optional and priced separately.
How long does a certified translation take?
Standard turnaround starts at 24 hours and depends on the length and language of your documents. If you have a deadline, include it in your quote request.
