Spanish vaccination record translation for a school, a civil surgeon or an employer, including the Mexican Cartilla Nacional de Salud and immunization cards from anywhere else in Latin America. Each dose, date and vaccine name is carried over exactly.
The Mexican cartilla is a booklet, not a certificate, and the vaccination pages are only part of it.
Which pages to send
The vaccine schedule sits on two or three pages of a booklet that also tracks weight, height and check-ups. Send the cover with the child's name and CURP and every page with a vaccine entry, including the adolescent and adult tables if they were used. Say whether you want the growth pages too; most recipients only ask for the doses.
Vaccine names and abbreviations
Mexican entries use the national schedule's names: BCG, Pentavalente acelular, SRP for sarampión, rubéola y parotiditis, which is MMR in the US, and DPT for the diphtheria, pertussis and tetanus booster. The translation gives the US name and keeps the original abbreviation in brackets so a nurse can match each line to the CDC schedule.
For the I-693 medical exam
A civil surgeon completing Form I-693 needs to see which vaccines you have already had before deciding what to give. Bring the translated record to the appointment with the original. Each dose is listed with its date as written, and handwritten entries or lot stickers that cannot be read are marked illegible rather than guessed.
Who is asking for your Spanish vaccination record translation?
The translation is the same. What changes is whether the recipient wants anything added to it.
USCIS
USCIS asks for a complete English translation and a signed certification from the translator. No notarization is needed.
Most ask for a certified translation they can read next to the original. Digital delivery as a PDF is usually enough; ask for a hard copy if they want one.
Three things that save a day of back and forth on a Spanish vaccination record.
01
Photograph each page flat and in daylight
Vaccination cards are small and often handwritten. A flat, well-lit photo of every page, including the cover, lets the translator read each date and lot number.
02
Tell us whose record it is
Children's cards often carry a nickname or no surname. Give us the full name as it appears on the passport so the translation matches the rest of the file.
03
Leave vaccine names and lot numbers to us
Trade names, lot numbers and stamps are copied exactly as written, and the headings around them are translated, so a clinic or school can read the schedule without guessing.
Details and pricing
What is included in a Spanish vaccination record translation
One price per page covers the translation and its certification. A page is up to 250 words, and the quote tells you the page count before you pay.
Everything on the vaccination record is translated, including stamps, seals and handwritten notes. Tell us the English spelling you use for each name when you order.
Mexican records write the day first, so 03/07/2019 is 3 July. The translation writes the month in words to remove the doubt, and keeps the original figures beside it when a handwritten date could be read two ways.
What if the cartilla is handwritten and faded?
Most are. The nurse writes the date and the lot number by hand and adds a stamp, and older booklets are creased and faded. Photograph each page flat in daylight, with the whole page in frame. Our translators read the hand, and anything that truly cannot be read is marked illegible instead of invented.
Do you translate immunization records from other Spanish-speaking countries?
Yes. A carné de vacunación from Guatemala or Colombia, or a Dominican tarjeta de vacunación, follows a different national schedule with its own vaccine names and abbreviations, and we translate each one against that country's schedule. The price is the same $25 a page.
How much does a certified translation cost?
Certified translation is $25 per page, with a page counted as up to 250 words. Notarization, a mailed physical copy and an apostille are optional and priced separately.
How long does a certified translation take?
Standard turnaround starts at 24 hours and depends on the length and language of your documents. If you have a deadline, include it in your quote request.
