Russian vaccination record translation for a US school or university, a civil surgeon completing Form I-693, or an employer, from the прививочный сертификат, the карта профилактических прививок форма 063/у, or the vaccination pages of a child's медицинская карта from Russia, Kazakhstan, Belarus or Uzbekistan. Each dose, date and vaccine is carried over as written.
The Russian schedule, the Национальный календарь профилактических прививок, uses its own vaccine names and its own shorthand, and the record also holds TB tests a US form has no row for.
The сертификат, форма 063/у and the school card
The сертификат о профилактических прививках, форма 156/у-93, is a small booklet the parent keeps, with a page per disease and a stamp per dose. The поликлиника keeps форма 063/у, the карта профилактических прививок, and copies the entries into the child's school card, форма 026/у. If the booklet is lost, the clinic prints a выписка from 063/у instead. Send whichever you have; a clinic выписка with a round stamp and the doctor's signature is translated the same way as the booklet.
АКДС, ЖКВ, ОПВ and the V and R marks
Entries use the Russian names: АКДС is the whole-cell diphtheria, pertussis and tetanus vaccine, АДС-М the reduced-dose Td booster, ОПВ and ИПВ the oral and inactivated polio vaccines, ЖКВ and ЖПВ the live measles and mumps vaccines, ВГВ hepatitis B. Doses are marked V1, V2, V3 for вакцинация and R1, R2 for ревакцинация. The translation gives the US name and keeps the Russian abbreviation in brackets, so a school nurse can match each line to the CDC schedule. Pentaxim or Infanrix Hexa from a private clinic is kept by brand.
Манту, Диаскинтест and the COVID certificate
A Russian record has a Манту line for every year from age one, with the induration in millimetres, and from age eight a Диаскинтест. These are tuberculin tests, not vaccines, and the translation labels them as tests with the size as written; a US civil surgeon may still order a blood test. The БЦЖ scar size noted in infancy stays too. A COVID-19 certificate from Госуслуги lists Спутник V as Гам-КОВИД-Вак with a QR code, and we translate it with the certificate number intact.
Who is asking for your Russian vaccination record translation?
The translation is the same. What changes is whether the recipient wants anything added to it.
USCIS
USCIS asks for a complete English translation and a signed certification from the translator. No notarization is needed.
Most ask for a certified translation they can read next to the original. Digital delivery as a PDF is usually enough; ask for a hard copy if they want one.
Three things that save a day of back and forth on a Russian vaccination record.
01
Photograph each page flat and in daylight
Vaccination cards are small and often handwritten. A flat, well-lit photo of every page, including the cover, lets the translator read each date and lot number.
02
Tell us whose record it is
Children's cards often carry a nickname or no surname. Give us the full name as it appears on the passport so the translation matches the rest of the file.
03
Leave vaccine names and lot numbers to us
Trade names, lot numbers and stamps are copied exactly as written, and the headings around them are translated, so a clinic or school can read the schedule without guessing.
Details and pricing
What is included in a Russian vaccination record translation
One price per page covers the translation and its certification. A page is up to 250 words, and the quote tells you the page count before you pay.
Everything on the vaccination record is translated, including stamps, seals and handwritten notes. Tell us the English spelling you use for each name when you order.
The entries are handwritten and some are only a stamp. Can you read them?
Usually. Each dose is a nurse's handwritten date, the серия or lot number, and a stamp of the clinic, and in older booklets the stamps overlap. Photograph each page flat, including the cover with the child's name and date of birth. Anything that cannot be read is marked illegible rather than guessed, because a civil surgeon or school nurse decides from those dates whether to repeat a dose.
How does the Russian schedule line up with what a US school asks for?
Mostly well, with two gaps. Russia gives measles and mumps as a combined дивакцина with rubella separate, which the translation records dose by dose so the nurse can count two of each. Varicella is not on the Russian national calendar, and hepatitis A and HPV only in regional ones, so expect those lines to be missing rather than mistranslated. Chickenpox had as an illness is sometimes written in the card as ветряная оспа перенесена, and we translate that note too.
Do you translate a US immunization record into Russian?
Yes. A child moving to Russia or Kazakhstan needs the record for a детский сад or school, where the clinic copies it into a new форма 063/у, and for the медицинская карта required before enrolment. We translate the US record into Russian with the Russian vaccine names beside the US ones, so the поликлиника nurse can see which calendar doses are done and which, like БЦЖ and Манту, Russia will ask about.
How much does a certified translation cost?
Certified translation is $25 per page, with a page counted as up to 250 words. Notarization, a mailed physical copy and an apostille are optional and priced separately.
How long does a certified translation take?
Standard turnaround starts at 24 hours and depends on the length and language of your documents. If you have a deadline, include it in your quote request.
