Chinese vaccination record translation to English for a school enrollment, a university health form, a civil surgeon's I-693 or an employer, from the 预防接种证 booklet every child in mainland China is issued as a baby. Each vaccine, dose, date and clinic seal is carried over, and nothing on the Chinese schedule is dropped because the US schedule lacks it.
Written for Chinese (Mandarin) Simplified vaccination record records
Reading the 预防接种证
The booklet is kept by the parents and stamped at each visit by the 接种单位, usually a 社区卫生服务中心 or a township 卫生院, so it is handwritten, stamped and ordered by vaccine rather than by date.
Vaccines a US nurse will not recognize
The national program (免疫规划疫苗) gives 乙肝 hepatitis B at birth, 卡介苗 BCG, 脊灰 polio as IPV and bOPV, 百白破 DTaP, 麻腮风 MMR, 乙脑 Japanese encephalitis, 流脑 meningococcal A and A+C, and 甲肝 hepatitis A. BCG, JE and the group A meningitis doses have no line on a US school form, and the 白破 booster at age six is diphtheria and tetanus without pertussis, so it does not count as a Tdap. We translate every line with the vaccine's full name and keep the Chinese abbreviation beside it so the nurse can decide what is still owed.
免疫规划 and 非免疫规划 pages
The free program vaccines and the paid ones (非免疫规划疫苗, once called 二类疫苗) are recorded on separate pages. Varicella, PCV13, rotavirus, flu, HPV and the 五联 pentavalent shot that replaces separate DTaP, IPV and Hib doses are paid vaccines in most provinces, so a child who had them has entries on the second table that a reader who stops at the first page will miss. Send every page with an entry. Where the nurse wrote the manufacturer and the 批号 lot number, we copy them exactly.
The 查验证明 and the electronic record
Kindergartens and primary schools check the booklet at enrollment, and the clinic issues a 预防接种证查验证明 listing what is complete and what is missing. If the booklet is lost, the clinic that gave the shots can reprint the record from the provincial immunization system, and many provinces now issue an 电子预防接种证 through a health app with a QR code. A reprint on clinic letterhead with a red seal translates cleanly; a phone screenshot does not, so ask the clinic for the printout.
Who is asking for your Chinese (Mandarin) Simplified vaccination record translation?
The translation is the same. What changes is whether the recipient wants anything added to it.
USCIS
USCIS asks for a complete English translation and a signed certification from the translator. No notarization is needed.
Most ask for a certified translation they can read next to the original. Digital delivery as a PDF is usually enough; ask for a hard copy if they want one.
Three things that save a day of back and forth on a Chinese (Mandarin) Simplified vaccination record.
01
Photograph each page flat and in daylight
Vaccination cards are small and often handwritten. A flat, well-lit photo of every page, including the cover, lets the translator read each date and lot number.
02
Tell us whose record it is
Children's cards often carry a nickname or no surname. Give us the full name as it appears on the passport so the translation matches the rest of the file.
03
Leave vaccine names and lot numbers to us
Trade names, lot numbers and stamps are copied exactly as written, and the headings around them are translated, so a clinic or school can read the schedule without guessing.
Details and pricing
What is included in a Chinese (Mandarin) Simplified vaccination record translation
One price per page covers the translation and its certification. A page is up to 250 words, and the quote tells you the page count before you pay.
Everything on the vaccination record is translated, including stamps, seals and handwritten notes. Tell us the English spelling you use for each name when you order.
Does a US school accept the booklet, or do I need a doctor's form?
Most districts and universities accept the translated record and have their own nurse transcribe it onto the state form. The translation gives each dose its own line with the vaccine name, the date and the clinic that stamped it, which is what the nurse copies from. Expect to be asked for a Tdap, a MenACWY and often varicella, since those are not part of the mainland schedule for most children.
The civil surgeon wants proof of each vaccine for Form I-693. Is the booklet enough?
Yes, if the entries are legible and carry the clinic stamp. A civil surgeon can count a dose only if the record shows the vaccine and the date, so where a vaccine name is blurred, the translation says so and the surgeon decides whether to repeat the dose. Bring the original booklet and our translation to the appointment. Adult records are rarer in China; if all you have is a 乙肝 course or a 新冠疫苗接种凭证, we translate that too.
Do you translate the yellow international certificate from the 国际旅行卫生保健中心?
Yes, though it rarely needs much. The 疫苗接种或预防措施国际证书 issued by a 国际旅行卫生保健中心 under the customs authority is printed in Chinese, English and French, so only handwritten Chinese entries and the seal need translating. The same office's 国际旅行健康检查证明书, with a physical exam and lab results, is mostly bilingual as well, apart from the doctor's handwritten findings.
How much does a certified translation cost?
Certified translation is $25 per page, with a page counted as up to 250 words. Notarization, a mailed physical copy and an apostille are optional and priced separately.
How long does a certified translation take?
Standard turnaround starts at 24 hours and depends on the length and language of your documents. If you have a deadline, include it in your quote request.
