Written for French vaccination record records

Reading the vaccination pages, country by country

Each French-speaking country records shots in a booklet the parent keeps, with its own schedule, vaccine names and abbreviations.

The French carnet de santé and the eleven obligatoires

In France the vaccination pages sit at the back of the carnet de santé, one line per injection: date, vaccine, numéro de lot, and the doctor's signature and cachet, often with the box's sticker pasted in. Children born from 1 January 2018 need eleven vaccines to enter a crèche or school, so a record for a child born since then normally shows all eleven by age two. The names differ from the CDC table: DTCaP or DTCaPolio is DTaP plus polio, ROR is MMR, and Hib, HepB, pneumocoque and méningocoque C each take one line. Many stickers name only the product, Infanrix hexa, Hexyon, Priorix, MMRVaxPro or Prevenar 13, so the translation adds the diseases it covers.

BCG, and the PEV card from West Africa

BCG was compulsory in France until 2007 and is on most adult records; it explains a positive TB skin test to a civil surgeon, who can order a blood test instead. Records from Senegal, Côte d'Ivoire, Cameroon, Guinea, Mali and Haiti come from the Programme Élargi de Vaccination as a folded carte or carnet de vaccination, where Penta is the DTC-HepB-Hib combination, VPO and VPI are oral and injected polio, and RR is measles and rubella without mumps. Travellers also hold the yellow fièvre jaune certificate, printed in French and English, so only its handwritten entries need translating.

Québec, Belgium and the shots France does not give

A Québec child's record is the carnet de vaccination, and the clinic also enters each dose in the Registre de vaccination du Québec, so a printout from Carnet santé Québec can stand in for a lost booklet. In Belgium the ONE carnet de l'enfant follows the Fédération Wallonie-Bruxelles schedule, which times some doses differently. Varicelle is not routine in France or Belgium, so a US school will ask for the shot or a titer; the translation lists what is on the page and never implies a dose the record does not show.

Who is asking for your French vaccination record translation?

The translation is the same. What changes is whether the recipient wants anything added to it.

USCIS

USCIS asks for a complete English translation and a signed certification from the translator. No notarization is needed.

USCIS translation requirements

Schools, clinics and employers

Most ask for a certified translation they can read next to the original. Digital delivery as a PDF is usually enough; ask for a hard copy if they want one.

Certified translations

US passports and the Department of State

The Department of State wants the translation notarized. Add notarization at checkout and we handle the rest.

Notarized translations

Before you upload

Three things that save a day of back and forth on a French vaccination record.

01

Photograph each page flat and in daylight

Vaccination cards are small and often handwritten. A flat, well-lit photo of every page, including the cover, lets the translator read each date and lot number.

02

Tell us whose record it is

Children's cards often carry a nickname or no surname. Give us the full name as it appears on the passport so the translation matches the rest of the file.

03

Leave vaccine names and lot numbers to us

Trade names, lot numbers and stamps are copied exactly as written, and the headings around them are translated, so a clinic or school can read the schedule without guessing.

French Vaccination Record FAQ

Most questions about pricing and turnaround can be answered faster by requesting a quote.

The entries are stickers, stamps and three different hands. How are they translated?
Line by line. French dates run day first, so 03/07/2019 is 3 July, and the translation spells out the month. A sticker is transcribed as printed, with the diseases named after the product, and the numéro de lot stays as it is. A cachet gives the doctor's name and RPPS number, and that is carried over too. Photograph each page flat and in full, stickers included, even the half-torn ones.
The carnet de santé is lost. What can I send instead?
Ask the médecin traitant or the pédiatre for an attestation de vaccination copied from the patient file; most French practices print one from their software. For a child, the PMI of the département can issue a duplicate carnet. Doses entered since 2022 may also appear in the vaccination section of Mon espace santé, and that printout is translated like any other page.
Do you translate a US immunization record into French for a crèche or school in France?
Yes. A crèche, an assistante maternelle or an école maternelle asks for proof of the vaccinations obligatoires at inscription, and a French doctor then copies the doses into a new carnet de santé. We translate the US record into French with the CDC names kept in brackets so the doctor can match each dose to the calendrier vaccinal. A Québec school or CPE may ask for the same record in French.
How much does a certified translation cost?
Certified translation is $25 per page, with a page counted as up to 250 words. Notarization, a mailed physical copy and an apostille are optional and priced separately.
How long does a certified translation take?
Standard turnaround starts at 24 hours and depends on the length and language of your documents. If you have a deadline, include it in your quote request.
