Written for Vietnamese vaccination record records

Reading a sổ tiêm chủng against the US schedule

Vietnam's Chương trình Tiêm chủng mở rộng gives a different set of vaccines at different ages from the CDC schedule, and the translation has to let a nurse see where the two line up.

The vaccines are named by disease

A Vietnamese record lists a vaccine by what it prevents: lao for BCG, viêm gan B, bạch hầu, ho gà, uốn ván for DTP, bại liệt for polio, sởi for measles, viêm não Nhật Bản for Japanese encephalitis. The five-in-one shot given at 2, 3 and 4 months is written as DPT-VGB-Hib or by brand, ComBE Five or the SII vaccine, and the oral polio drops as OPV with the injected dose as IPV. We give the US name and keep the Vietnamese and the brand in brackets so each line can be matched to the CDC table.

The 9-month measles dose and what the US counts

The state programme gives a single-antigen measles shot at 9 months and a measles-rubella dose at 18 months. A US school or civil surgeon counts only doses given on or after the first birthday, and neither state dose contains mumps, so a child with a complete Vietnamese record is often still asked for MMR. Private clinics give Priorix or MMR II, and if the booklet shows one, the translation makes that plain. We never relabel a sởi dose as MMR.

Paper book, private booklet and the app

A trạm y tế record is a small printed book with handwritten dates, the nurse's initials and a stamp. VNVC and the Viện Pasteur issue their own booklet listing the brand, the lot and the next due date, and the Sổ Sức khỏe điện tử app shows the same history on a screen. Send clear photos of every page with a date on it, or screenshots of each entry in the app, and we translate them as one record. Dates are day first, and the translation writes the month in words.

Who is asking for your Vietnamese vaccination record translation?

The translation is the same. What changes is whether the recipient wants anything added to it.

USCIS

USCIS asks for a complete English translation and a signed certification from the translator. No notarization is needed.

USCIS translation requirements

Schools, clinics and employers

Most ask for a certified translation they can read next to the original. Digital delivery as a PDF is usually enough; ask for a hard copy if they want one.

Certified translations

US passports and the Department of State

The Department of State wants the translation notarized. Add notarization at checkout and we handle the rest.

Notarized translations

Before you upload

Three things that save a day of back and forth on a Vietnamese vaccination record.

01

Photograph each page flat and in daylight

Vaccination cards are small and often handwritten. A flat, well-lit photo of every page, including the cover, lets the translator read each date and lot number.

02

Tell us whose record it is

Children's cards often carry a nickname or no surname. Give us the full name as it appears on the passport so the translation matches the rest of the file.

03

Leave vaccine names and lot numbers to us

Trade names, lot numbers and stamps are copied exactly as written, and the headings around them are translated, so a clinic or school can read the schedule without guessing.

Will a US school accept the translated sổ tiêm chủng?
Schools and daycares take a certified translation of the record and have a nurse or a doctor transcribe it onto the state immunization form. Most states accept a foreign record if the vaccine, the date and the child's name are legible. Where the Vietnamese schedule stops short of the US one, the school asks for catch-up doses, and the translation lets the nurse see exactly which.
My COVID-19 certificate is on the PC-Covid or Sổ Sức khỏe điện tử app. Can you translate that?
Yes. The certificate shows the vaccine, the dose number, the date and the vaccination site, with a QR code. Send a screenshot of the full certificate for each dose. Doses of Vero Cell, AstraZeneca, Pfizer or Moderna are translated by brand as printed, and the US recipient decides which ones it recognizes.
Do you translate a US immunization record into Vietnamese?
Yes. A child moving to Vietnam needs the record for a trường mầm non or for a Vietnamese clinic to pick up the schedule, and many parents want a translated copy for the trạm y tế. We translate the CDC-format printout or the state registry record into Vietnamese, naming each vaccine the way a Vietnamese nurse writes it, so Hib, PCV and MMR are not left as unexplained initials.
How much does a certified translation cost?
Certified translation is $25 per page, with a page counted as up to 250 words. Notarization, a mailed physical copy and an apostille are optional and priced separately.
How long does a certified translation take?
Standard turnaround starts at 24 hours and depends on the length and language of your documents. If you have a deadline, include it in your quote request.
