Ukrainian medical records translation to English for a US physician continuing treatment, a health insurer, a disability claim, a personal injury lawyer or an asylum file. We translate the виписка з медичної карти, the епікриз, the hospital історія хвороби, disability certificates and the records of military hospitals, from paper, from a doctor's handwriting or from an eHealth printout.
Ukrainian medical paperwork is built from numbered ministry forms, and a diagnosis is written in a shorthand of Ukrainian, Russian and Latin that a US clinician cannot read with a dictionary.
The forms a Ukrainian patient leaves with
A hospital stay ends with a виписний епікриз or a виписка з медичної карти стаціонарного хворого, form 027/о, which gives the diagnosis, the course, the operations and the discharge advice on one or two pages. Outpatient care sits in the медична карта амбулаторного хворого, form 025/о, a card filled by hand over years. For a school, a job or a university there is the довідка 086/о, a fitness certificate. We translate the form number and title as printed, so a US doctor knows which record it is, and the handwritten entries line by line.
Diagnoses in three languages
A Ukrainian diagnosis line reads like ІХС, стенокардія напруги II ФК, ГХ II ст., with the МКХ-10 code beside it and Latin for anatomy and prescriptions: Sol. Analgini 50% 2 ml. The abbreviations are expanded in the translation, ІХС to ischemic heart disease and ГХ to hypertension, and the МКХ-10 code is kept as the ICD-10 code it is. Drug names are given as the Ukrainian brand with the active substance beside it, because a US pharmacist will not recognise Корвалол or Цитрамон by name.
Disability certificates and military records
Disability in Ukraine was set by a МСЕК commission until 2025 and since then by an експертна команда at a hospital, as група інвалідності I, II or III with a date for re-examination. The довідка has no US equivalent, so the translation keeps Group II disability and explains the scale in a note for an SSA examiner or an insurer. Since 2022 many files include records from a військовий госпіталь or a ВЛК, the military medical commission, whose conclusion states fitness for service. Those use the same forms under military headings, and ranks and unit numbers are translated as printed.
Who is asking for your Ukrainian medical records translation?
The translation is the same. What changes is whether the recipient wants anything added to it.
Doctors and hospitals
A new physician needs your diagnoses, medications and reports in English before the first visit. Each report is translated in full and certified, so it can go into your chart as it is.
Health, travel and disability insurers ask for the medical report and the itemized bill in English, translated completely and certified, before they pay a claim for care received abroad.
ECFMG accepts a word-for-word translation on a translation service's letterhead, signed by the translator with a certification statement. Summaries and extracts are refused. Our certified translations follow that format.
Three things that save a day of back and forth on a Ukrainian medical records.
01
Send the pages a doctor would read
The discharge letter, the current medication list and the reports it relies on cover most visits. A full hospital file can run to hundreds of pages, so start with those and add the rest if the recipient asks.
02
Letterhead, stamps and signatures count
The hospital header, the doctor's stamp and the registration number on a prescription identify who wrote the record. Scan each page whole, including forms and the back of any page with a stamp.
03
Tell us who will read it
A physician needs the clinical pages, an insurer the diagnosis and the itemized bill, ECFMG a word-for-word translation of the entire document. Say which in the order notes, with the patient's name as the passport spells it.
Details and pricing
What is included in a Ukrainian medical records translation
One price per page covers the translation and its certification. A page is up to 250 words, and the quote tells you the page count before you pay.
Everything on the medical records is translated, including stamps, seals and handwritten notes. Tell us the English spelling you use for each name when you order.
I need my records for a US doctor, not an agency. Do I still need a certified translation?
A doctor does not need the certification, but it is included anyway, and the insurer handling the same care may ask for it later. What matters for the doctor is completeness: send the whole епікриз rather than the last page, the lab printouts with their reference ranges, and any imaging report, since Ukrainian ranges and units differ from US ones and the translation keeps the units as printed with the US unit in brackets where it helps.
Can you translate records I printed from eHealth or from Helsi?
Yes. Since 2018 Ukrainian clinics record visits in the electronic health system, and a patient can download a visit summary, a prescription or a referral from Helsi or another connected app, each with a reference number. The printout is typed, which reads better than a handwritten карта, and we translate it with its reference numbers and the doctor's name and facility as shown.
Do you translate the whole hospital file or a summary?
Whatever you send, page by page, and we do not summarise. For most purposes the епікриз is enough, since it was written to sum up the stay. A personal injury or asylum lawyer may want the full історія хвороби, the inpatient chart, which can run to dozens of handwritten pages at $25 each, so ask the lawyer which pages matter before uploading the whole chart.
How much does a certified translation cost?
Certified translation is $25 per page, with a page counted as up to 250 words. Notarization, a mailed physical copy and an apostille are optional and priced separately.
How long does a certified translation take?
Standard turnaround starts at 24 hours and depends on the length and language of your documents. If you have a deadline, include it in your quote request.
