Ukrainian vaccination record translation for school enrollment, a civil surgeon's I-693 exam or the Uniting for Ukraine vaccine attestation. We translate the карта профілактичних щеплень, the clinic's довідка про щеплення and printouts from the electronic health system, dose by dose, with the Ukrainian vaccine names matched to the ones a US nurse uses.
Ukraine has no single national vaccination card. The record lives in the clinic's files, and what a parent carries is a copy or an extract of it, which looks different from one clinic and one decade to the next.
Форма 063/о and the довідка
The карта профілактичних щеплень, форма 063/о, is the clinic's own card, filled in by hand by each nurse who gave a dose. For a school or a kindergarten the clinic issues a довідка про щеплення, a one-page extract signed by the pediatrician and stamped by the поліклініка. Older children often have both, plus a page from the історія розвитку дитини. Send whichever you hold; we translate the card, the extract and every stamp on it.
БЦЖ, АКДП, КПК and the Mantoux line
Ukrainian entries use the national calendar's abbreviations: БЦЖ is BCG, АКДП or АаКДП is DTP or DTaP, ОПВ and ІПВ are oral and inactivated polio, КПК is measles, mumps and rubella, so MMR, and ВГВ is hepatitis B. The translation gives the US name and keeps the original abbreviation in brackets. The same card records проба Манту, the tuberculin skin test, with the induration size in millimetres; we translate it as a test result, not a vaccine, because a US form lists it separately.
Doses the US schedule expects and Ukraine does not give
Varicella, pneumococcal and meningococcal vaccines are not on Ukraine's routine calendar, so a complete Ukrainian record still leaves gaps on a US school form. A school nurse or civil surgeon reads the translated record to decide what to give, which is why each date is carried over exactly as written, in the day.month.year order the clinic used, with the month spelled out beside it.
Who is asking for your Ukrainian vaccination record translation?
The translation is the same. What changes is whether the recipient wants anything added to it.
USCIS
USCIS asks for a complete English translation and a signed certification from the translator. No notarization is needed.
Most ask for a certified translation they can read next to the original. Digital delivery as a PDF is usually enough; ask for a hard copy if they want one.
Three things that save a day of back and forth on a Ukrainian vaccination record.
01
Photograph each page flat and in daylight
Vaccination cards are small and often handwritten. A flat, well-lit photo of every page, including the cover, lets the translator read each date and lot number.
02
Tell us whose record it is
Children's cards often carry a nickname or no surname. Give us the full name as it appears on the passport so the translation matches the rest of the file.
03
Leave vaccine names and lot numbers to us
Trade names, lot numbers and stamps are copied exactly as written, and the headings around them are translated, so a clinic or school can read the schedule without guessing.
Details and pricing
What is included in a Ukrainian vaccination record translation
One price per page covers the translation and its certification. A page is up to 250 words, and the quote tells you the page count before you pay.
Everything on the vaccination record is translated, including stamps, seals and handwritten notes. Tell us the English spelling you use for each name when you order.
Which vaccination record do I need for the Uniting for Ukraine attestation?
The attestation covers measles and polio, and earlier also COVID-19. Those doses are in the childhood card or the clinic's довідка; a COVID-19 certificate, if you still need one, comes from Diia. Send what you have and we translate it, so the dates you enter on the USCIS form can be checked against the originals if anyone asks.
Our record is a printout from Helsi or the clinic's electronic system. Is that enough?
Usually, yes. Since vaccinations have been entered in the national electronic health system, a family doctor can print a list of doses with the vaccine name, date and batch. Doses given before the system existed may be missing from it, so compare the printout with the paper card and send both if they differ. We translate each and note which one each dose came from.
Will the civil surgeon count the doses on a translated Ukrainian card?
A civil surgeon counts a dose from a written record when the record gives its date. Entries that say only щеплений, vaccinated, with no date are the usual problem on older Ukrainian cards, and for those the doctor gives the vaccine again or orders a blood test for immunity. We translate the undated entries as written so the doctor can see exactly what the card says.
How much does a certified translation cost?
Certified translation is $25 per page, with a page counted as up to 250 words. Notarization, a mailed physical copy and an apostille are optional and priced separately.
How long does a certified translation take?
Standard turnaround starts at 24 hours and depends on the length and language of your documents. If you have a deadline, include it in your quote request.
